چکیده

Objective: To detect occurrence of T-wave and QRS alternans, to evaluate their independent predictors, and to estimate alternans change during haemodialysis (HD). Methods: We studied 58 patients, mean age 59 ± 13 years, 52% males. We performed 1-minute ECG-recording before and after HD determining episodes of alternans. Results: T-wave alternans episodes were present in 21% and 28% of patients pre-HD and post-HD, respectively, without significant change during the procedure. Pre-HD values were independently predicted by body mass index (BMI) (negative correlation) and number of cigarettes smoked (positive correlation). Post-HD values were predicted by usage of acetyl salicylic acid (ASA) and ferric substitutes (negative correlation) and baseline heart rate (positive correlation). Episodes of QRS alternans were evident in 7 patients pre-HD and one post-HD: significant decrease during HD (from 0.26 ± 0.85 to 0.07 ± 0.53 number of alternans episodes, p = 0.015). Independent predictors for baseline QRS alternans values were BMI and ASA usage (negative correlation), baseline heart rate and calcium channel blocker usage (positive correlation). Conclusion: Both T-wave and QRS alternans are present in HD-patients. T-wave alternans episodes did not change significantly during HD-procedure, while episodes of QRS alternans showed significant decrease. Regression analysis suggested protective effects for higher BMI and ASA usage in this group.

Electrical alternans of the T-U wave without change in the QRS complex.

A patient with myelocytic leukemia who showed electrical alternans of the T-U wave with no change in the QRS complex following chemotherapy is described. Electrocardiogram taken 4 days later showed ventricular quadrigeminy in which the T-U wave of the first sinus beat after the ventricular premature contraction was markedly less prominent compared to the successive two sinus beats which showed ...

Macroscopic Visualization of the Heart Electrical Activity Via an Algebraic Computer Model

In this study, a mathematical model is developed based on algebraic equations which is capable of generating artificially normal events of electrocardiogram (ECG) signals such as P-wave, QRS complex, and T-wave. This model can also be implemented for the simulation of abnormal phenomena of electrocardiographic signals such as ST-segment episodes (i.e. depression, elevation, and sloped ascending...

Microvolt T-wave alternans distinguishes between patients likely and patients not likely to benefit from implanted cardiac defibrillator therapy: a solution to the Multicenter Automatic Defibrillator Implantation Trial (MADIT) II conundrum.

BACKGROUND In 2003, the Centers for Medicaid and Medicare Services recommended QRS duration as a means to identify MADIT II-like patients suitable for implanted cardiac defibrillator (ICD) therapy. We compared the ability of microvolt T-wave alternans and QRS duration to identify groups at high and low risk of dying among heart failure patients who met MADIT II criteria for ICD prophylaxis. M...

A Unified Framework for Delineation of Ambulatory Holter ECG Events via Analysis of a Multiple-Order Derivative Wavelet-Based Measure

In this study, a new long-duration holter electrocardiogram (ECG) major events detection-delineation algorithm is described which operates based on the false-alarm error bounded segmentation of a decision statistic with simple mathematical origin. To meet this end, first three-lead holter data is pre-processed by implementation of an appropriate bandpass finite-duration impulse response (FIR) f...

Independent predictive accuracy of classical electrocardiographic criteria in the diagnosis of paroxysmal atrioventricular reciprocating tachycardias in patients without pre-excitation.

AIMS In patients without pre-excitation, the differential diagnosis of paroxysmal atrioventricular (AV) reciprocating tachycardias consists mainly of atrioventricular nodal re-entrant tachycardias (AVNRTs) and AV reciprocating tachycardias (AVRTs) through a concealed bypass. Our purpose was to validate the diagnostic accuracy of a predictive logistic model using classical electrocardiographic (...
